
First — my dog is named Cloud. He’s a beagle, and honestly, he inspires me to be the person he already believes I am. That usually means showing up consistently, listening well, staying curious, and always bringing snacks whenever possible.
I’m Timothy Hamn, a board-certified Family Nurse Practitioner and Certified Wound Specialist providing personalized primary care, chronic disease management, and advanced wound care services throughout the Pacific Northwest. My practice is built on the belief that good medicine starts by listening — taking the time to understand each patient’s story, lifestyle, concerns, and goals before creating a treatment plan tailored specifically to them.
I provide comprehensive care for patients with both routine and complex medical needs, including diabetes, hypertension, lipid disorders, medication management, preventive care, skin and wound conditions, and mobility-related health concerns. I also specialize in advanced wound care, including pressure injuries, diabetic foot ulcers, vascular wounds, lymphedema-related wounds, skin integrity management, compression therapy, and debridement when appropriate.
My approach combines evidence-based medicine with practical, compassionate care focused on improving quality of life and long-term health outcomes. Whether helping patients streamline medications, manage chronic conditions, heal difficult wounds, or navigate everyday health concerns, my goal is to provide thoughtful, unhurried care that emphasizes education, continuity, and collaboration.
I offer both mobile medical visits and office-based care to improve accessibility for patients across Oregon and the Pacific Northwest — particularly those with mobility limitations, medically complex conditions, or difficulty accessing traditional healthcare settings.
I believe in treating the person — not just the diagnosis. Every patient brings unique experiences, challenges, and insight into their own health, and I believe listening carefully is one of the most important parts of medicine. My goal is to work collaboratively with patients to restore balance, reduce unnecessary medications when appropriate, promote independence, and build sustainable long-term health.
